Google Forms – Forms & Surveys in Confluence

Embed using the form URL

1. Click on the “Send button” for your form in Google Forms

2. Click on the “link icon” in the Send via menu

3. Click on the “Copy” button

   • Note: Do not check the “Shorten URL” checkbox

4. Paste the URL into the Confluence editor

   • If needed, change the width and height of the macro

5. Publish the Confluence page

Embed using the macro editor

1. Edit the Confluence page

2. Type /googleforms and hit enter

3. Add the embed URL and if needed, change the width and height of the macro

4. Publish the Confluence page
